1609-PR Image

Per Surplus Lines Law Section 1609, a written declaration from the producer is required (1609-PR). The surplus lines licensee must upload a signed 1609-PR image when submitting a 1609-SLL filing. PSLA reviews the uploaded 1609-PR image to insure it is consistent with the 1609-SLL data entered into EFS. If discrepancies are found, the filing state is set to “Filing Returned” in EFS (Electronic Filing System) and an email notification is sent to the Filing Return contact listed in EFS. The agency must revise/submit a corrected 1609-SLL filing with associated 1609-PR image.

Below is a list of common discrepancies with submitted 1609-PR images. If the discrepancy is on the 1609-PR image, please have the WRITING PRODUCER provide you with a corrected 1609-PR Producer Affidavit.

  1. The policy number or customer ID on the 1609-PR image does not match the data entered in EFS.
  2. The spelling of the named insured on the 1609-PR image does not match the spelling of the data entered in EFS.
  3. The named insured is missing on the 1609-PR.
  4. The named insured on the 1609-PR image does not match the data entered in EFS.
  5. The location of risk on the 1609-PR image is missing or does not match the data entered in EFS.
  6. The location of risk cannot be P.O. Box number.
  7. The location of risk cannot read "various." If there is more than one location for the specified policy, please provide the address with the most exposure.
  8. The type of coverage (Kind) on the 1609-PR is missing or does not match the data entered in EFS.
  9. The type of coverage (Kind) is not applicable for the limit(s) you have selected.
  10. The description of insured's operation (Class) on the 1609-PR image is missing or does not match the data entered in EFS.
  11. The policy effective and/or expiration date does not match the date entered in EFS.
  12. The property or casualty limits on the 1609-PR image does not match the data entered in EFS.
  13. The license number of the individual Producer is not a valid P&C license number. Use PID's Licensed Producer Search by License Type in order to find the correct P&C producer license number. If this is a producer license with limited Lines of Authority (powers) of "Personal Lines" and you believe it is valid for this filing, please email a filing copy of the policy to DataServices@pasla.org and state in the email that this Personal Lines producer license number is valid for this filing.
  14. The name of the Individual Producer is missing and/or does not match.
  15. The signature of the Producer is missing and/or is not legible.
  16. The image is not legible and PSLA cannot view it.
  17. The attached image is not a 1609-PR.
  18. The uploaded 1609-PR image does not match the data entered in EFS.
  19. The NAIC number(s) for the declining licensed insurer is missing or is not a licensed P&C insurer.
  20. The attached 1609-PR image uses the old form and it cannot be used on filings with a policy effective date on or after 1/1/2013. Please obtain an updated 1609-PR from your producer and upload it.
  21. The attached 1609-PR is not the prescribed affidavit per the Department.
  22. If this is a multi-year placement, the policy information must be attached with the 1609-PR.
  23. If you signed electronically, you must complete the Electronic Signature 1609-SLL Addendum to the Declaration and upload it with the 1609-PR.
